Update - We are continuing to work on a fix for this issue.

Identified - We have identified an issue affecting some client hierarchies within the our HX Foundations application where facilities may appear outside of their correct health system. This is a mapping-only issue and does not impact data integrity, access permissions, or reporting functionality.

Our technical team is actively working to correct the hierarchy mappings to ensure all facilities are accurately aligned with their respective health systems.

Resolved - We identified an issue affecting round entry between 2:30 p.m. and 2:45 p.m. EST, during which users may have been unable to progress through the round workflow. The incident has been resolved, and normal functionality has been restored.

Scheduled - iRound's next product upgrade will take place on Thursday, October 30th, between 3 and 5am ET. During this scheduled downtime, users will not be able to access iRound to collect or view data. All HL7 ADT Messages will queue during this time and will process in the order received when iRound is back online.

Resolved - Transparency application performance has been restored to normal and all pending scheduled reports have been delivered.

Update - The Transparency application has continued to perform normally. Scheduled reports have been delivered with the exception of the CPR reports, which are still currently on hold. Our technical team is continuing to work on a solution to allow these reports to be delivered.

Monitoring - Our technical team has implemented some changes and we are seeing improved performance. We are continuing to monitor.

Investigating - Users may be experiencing slow performance when accessing our Transparency comment approval application. Also, scheduled reports for Transparency, Reputation and Campaigns are on hold until performance is stabilized. Our technical teams are currently working with AWS Support to investigate the issue.

Investigating - Our technical teams are investigating an issue with Employee Experience dashboards on our HX Platform. Users are seeing error messages when attempting to access reports. We will provide additional updates as more information is known.

Scheduled - The next Press Ganey system maintenance will take place on Saturday, October 25, 2025 from approximately 6-10 pm ET.

During the scheduled downtime window, iRound will be unavailable to collect or view data. For clients sending HL7 messages, these messages will queue and begin processing once iRound is back online.
